About the Centre

The Oaks Centre in St Ann's, the fully accessible home, is small, friendly, and an integral part of the community. Providing residential care for the elderly and frail people and specialising in care for people with Dementia.

It is close to local amenities such as shops, a market, a library, a community centre, a Mosque and Churches, Stone Bridge City Farm, and local parks. It has excellent transport links to the city centre, which is only one mile away. Residents are supported by staff to access the local amenities to encourage independence.

 

A list of our services:

  • All our carers are fully trained and provide our residents 24-hour support. 
  • We are fully compliant with the Care Quality Commission (CQC) 
  • We are also audited by Nottingham City Council's Quality Monitoring Team and NHS Nottingham City Clinical Commissioning Group on the quality of our services.
  • We use the Telecare system, which ensures safety and privacy

We work with various health professionals to offer the best care for our residents. We maintain close contact with the Dementia Care Team, Outreach Team and Falls Prevention Team, as well as Occupational Therapists, Physiotherapists, GPs and a range of other health professionals. 

Residents are assigned a key worker during their stay who will work closely with them to assess their needs. We aim to support our residents to work towards their goals and to help them maintain their independence.

We provide person-centred support tailored to meet each resident's individual needs. Having this at the heart of our residential home enables us to offer diverse activities that cater to our residents varying and ever-changing needs. The team are committed to finding new and exciting ways to enable residents to lead full and active lives whilst supporting them to retain their independence.

We also work closely with outside agencies such as Age UK, Money Carers Foundation and POhWER to benefit our residents.

  • En suite facilities - Our friendly home offers all residents their private single room for comfort and convenience. We want to make our residents stay as comfortable as possible so home comforts, including furniture can be brought in.  
  • Grow and Eat Project, which uses the on-site allotment and provides fresh produce to use in meals and snacks throughout the day.
  • The menu is informed by residents, who are given the opportunity to have their own input into what food is provided.
  • Quiet and personal areas
  • Social and games Areas. - Offering a chance to participate in more social activities.

Family and friends are welcome anytime during the day, and we can arrange private spaces for residents and their families. There is also a relaxing café area staffed entirely by volunteers from the local area. 

Activities

We provide a timetable of activities which our residents inform in monthly meetings.

  • Table top activities (crafts and games)
  • Themed events,
  • Tea dances,
  • Bingo,
  • Movie nights
  • Visits from local community groups.
  • Visit to local amenities for shopping and pub lunches,
  • Opportunities to attend local church services
  • Organise trips out to the cinema, theatre and sporting events.
  • Hairdresser and chiropodist that visit the site.

As our care is person-centred we try to accommodate individual requests by residents and arrange the activity they would like to enjoy.
