Where to get help
Nottingham City Council's Adult Social Care Services department is responsible for assessing the needs of and providing support to, older, disabled and vulnerable people in the community.
For most people, social care won't involve social services. Only people with higher levels of need will be eligible for the services we provide.
By directing citizens and providers to the most appropriate advice, care and support we aim to help individuals maintain or recover their independence and promote their wellbeing.
If you need advice about care options you can ask Adult Social Care Services for an assessment by calling the Nottingham Health and Care Point.
Everyone is entitled to one, irrespective of financial circumstances, and they are free. Some people refer themselves but many are referred by their doctor, the hospital, family, friends or neighbours.
An assessment may involve someone from Adult Social Care Services speaking to you over the phone or visiting you, either at home or in hospital, to discuss your situation and work out what help would be appropriate to meet your social care needs.

Who will pay for my support services?
If you are eligible for services two assessments will be carried out. The first determines what your care needs are.
The second looks at your financial circumstances, this defines how much you will have to contribute to any care you receive.
If you choose not to have a financial assessment the council will assume that you can pay for all of the costs yourself.
A financial assessment is usually carried out by an officer from the Fairer Charging Team. They will look at:
- Your income
- Any savings and investments you may have
- Additional property
- Essential expenditure, e.g. rent or mortgage, council tax
- Disability related spending

Integrated Care
NHS Nottingham City Clinical Commissioning Group, Nottingham City Council and Nottingham CityCare Partnership are working together to integrate adult health and social care services as part of a five-year programme to improve and streamline care for citizens.
Integrated care aims to 'join up' health and social care to meet the needs of an ageing population and transform the way that care is provided for people with long-term conditions, enabling people with complex needs to live healthier, fulfilling and independent lives.
