Here you will find the application form and notices of application required to be completed should you wish to vary a premises licence issued under the Gambling Act 2005.

• Form A is to be used if there is one applicant only.
  • Notice of App for Premises Licence Form A (used when one applicant)

• Form B is to be used if there is more than one applicant. 
  • Notice of App for Premises Licence Form B (used when more than one applicant)

• Please see Gambling Act 2005 fees for the current application fees.

There will be an annual fee payable in respect of the premises licence issued by the licensing authority. Failure to pay the annual fee will result in the request being revoked.
