If you are clinically extremely vulnerable, the government has issued practical steps to help minimise your risk of exposure to COVID-19 wherever possible to stay safe:
- Limit social interactions, including mixing with other households.
- Reduce the amount of time you spend in settings where you are unable to maintain social distancing.
- Continue working from home if possible. If you cannot work from home, you should now attend your workplace. Your employer is required to take steps to keep you safe at work.
- Continue to access the NHS services that you need.
Guidance for those who are currently shielding has also been issued about the COVID-19 vaccination:
- If you are aged 16 or over, you should already have been offered your first dose of the vaccine. If you have not yet received your first dose, please contact your GP.
- If you have received your first dose, make sure that you get your second dose when it is offered to you, to further increase your level of protection.
- Even if you have had your vaccination, you should continue to take extra precautions to stay safe.

If you are Clinically Extremely Vulnerable and you have not registered for support before, then please use the form below to register yourself or someone else. If you need to register by phone, or you need support urgently, contact the Council directly on 0115 915 5555.
When you register, you will be asked for your NHS number. You can find it on any letter the NHS has sent you, or on a prescription. It is helpful if you register even if you do not have any support needs at this time. You can log in and update your needs if circumstances change at any time.

People who are Clinically Extremely Vulnerable are advised to:
- Try to keep contact with people outside your household, or support bubble, to a minimum and avoid busy areas.
- Whenever you go out, continue to maintain strict social distancing, wash your hands regularly and avoid touching your face.
- You should also try to stay 2 metres away from other people within your household, especially if they display symptoms of the virus or have been advised to self-isolate.

Digital Needs – We can help you register on the Government’s portal and for supermarket priority slots, and support with online activities.
Advice – We can put you in touch with services to provide advice on financial and employment concerns, guidance on understanding the tiered alert level restrictions and social distancing, help with substance abuse.
Up to date details – If you’ve had a letter from the Government advising you are Clinically Extremely Vulnerable, we can make sure your information and support needs are up to date and registered on the Government’s system.
Self-isolation is when you do not leave your home because you have or might have coronavirus.
If you have coronavirus symptoms or have tested positive for coronavirus you must self-isolate for 10 days.
You can get help with everyday tasks, like collecting shopping or medicines. Please contact the Customer Hub (Mon-Fri) on 0115 915 5555 and they will help you get practical support from local voluntary and community organisations
