Gateway Online Assessment
Supporting Nottingham People and Preventing Homelessness
As part of the Homelessness Prevention Gateway project, an online assessment process has been developed to make referrals quicker and easier. This online assessment is for use by day centres, advice agency's or hostels in making referrals rather than by individuals and is designed to cut down on the amount of paper work, administration and follow up enquiries. Paper and electronic versions of the information entered can be produced for agency's to hold on file.

Accessing the Gateway Online Assessment:

Login and password details will be required and can be obtained by emailing the Gateway team with the subject line "Online Contact Assessment: Login".
The online assessment is designed to ask follow up questions based on the information provided and to obtain relevant information in order make an accurate assessment. Given the broad range of circumstances that may arise, efforts have been made to capture information without being too onerous, however some individuals have complex needs that require detailed information. Development is still ongoing with the assessment and any feedback on issues, errors or improvements are more than welcome and should be emailed to the Gateway team with the subject line "Online Contact Assessment: Feedback". The objective of the online assessment is to make the assessment process as efficient and effective as possible.
