What we do

The team provides a service for people aged 18-65* with a neurological condition. We enable adults to live in the environment of their choice with appropriate sensitive and effective care plans by working with service users, agreeing with them and their carers the best way of meeting their needs or receiving support.

Where?

We are a community based team who can visit people at home, in day centres, residential establishments, hospital or other community settings within the Nottingham City Boundary.

* Our Senior 1 Neurological Physiotherapists also provide a service for over 65's within the city boundary.

What can we provide

• Information and advice on direct payments and independent living fund
• Information and guidance on daily living.
• Information on healthy lifestyles
• Support to families and carers
• Advice to other services and teams
• Rehabilitation and promotion of independence
• Signposting to approriate services - eg housing, employment options, and health services.
• Specialist adaptations and equipment.

Assessment

We can provide assessment of needs that may lead to the provision of Homecare Packages, Direct Payments, Respite,Residential Care, Day Care, Rehabilitation, and Equipment & Adaptations .The Team liaise closely with Health Care Professionals, in particular Rehabilitation Services and the City Stroke Team.

Assessments are undertaken holistically and using a person centred approach in order to agree in partnership with service users and their carers how best to meet individual need.

We also have a bungalow that is fully adapted to meet a wide range of physically and sensory impairment needs. People are able to practise developing independent living skills, spend time overnight  in the flat or just experience accomadation that has been adapted.
The Team work very closely with other health professionals, particularly  the City stroke Team and the Neurological clinicsbased in the Nottingham University Hospital. We have strong relationships with specialist housing and care providers and work in partnership with supporting people

We have a open referral system, which means we will take referrals from:

Self referrals
Clients/carers/family
GP
Community based practitioners.
All health and social care professionals
Other agencies (Voluntary and statutory)
Hospital
