Sending the referral form

The completed referral form, signed by the legal guardian, can be sent to the local Speech and Language Therapy team.

Send the completed referral form (signed by the legal guardian) to the Speech and Language Therapy team that works with the child's school or is closest to the child's home address. The form can be posted, faxed, delivered or given to the Speech and Language Therapy team. At present, it is not possible to email referral forms to Speech and Language Therapy as they contain confidential patient-identifiable information and need a legal guardian signature.

It is recommended that referrers keep a copy of any referrals made for their own records.

What happens after referral?

Once the referral is received and logged, a letter is sent to the child's parent(s)/ carer(s), asking them to telephone the central booking line on 0115 883 5196 within 2 weeks of receiving the letter to make an appointment. The parent can then choose a date, time and location that suits them from the choices available. A copy of this opt-in letter is sent to the referrer for their records. An information leaflet is also given:

If no contact is made from parent(s)/carer(s), a letter is sent to the referrer to let them know, and the referral is ended. The child can be re-referred at anytime.
