Accessing support from Speech and Language Therapy (SLT)

Anyone can refer to SLT as long as parents/carers consent.

Nottingham Children's Speech and Language Therapy Service has an open referral system, which means that any key person in the child's life is able to refer, as long as the child's legal guardian has given permission. This is why there must be a parent/carer signature on the referral form.

The Speech and Language Therapy Service receives referrals from many different people, including the child's parent(s)/carer(s), teachers, SENCOs, teaching assistants, GPs, Paediatricians, Educational Psychologists, ENTs, Health Visitors, Nursery teachers, Nursery Nurses, Inclusive Education Services, Social Workers.  Parents may need help from school professionals to complete this form.

This document is a read-only Adobe PDF, and must be printed out and signed by the legal guardian before sending to Speech and Language Therapy.  Please be aware that Speech and Language Therapy may not be able to process this referral if it is incomplete, and it will be returned to the referring agency to complete any missing information.
