If you live in Nottingham City you must apply to Nottingham City Council for a school place. You can't apply to any school directly unless it's an independent school. If you don't live in Nottingham City, you should contact your home local authority, even if you want to apply for a Nottingham City school.

Faith-based schools normally have a supplementary form that you should also complete (see below).

If you have not changed address, you are strongly encouraged to keep your child at their present school unless they can no longer get there.

Secondary school supplementary forms

Supplementary forms should be completed if making an application for secondary education in the 2012/13 year for:
