We use assessments to help us identify your needs. Once we know your needs, we can then discuss with you the different ways in which your needs may be met. This may include providing or arranging services for you, if you are eligible for them. We cannot provide services to everyone so we use eligibility criteria alongside your assessment to help us decide if you are eligible for services.

If you are eligible for services and you would like to receive them, we will draw up a care plan which will detail what services you will receive.
